醒脑开窍针刺法联合电子生物反馈治疗脑卒中后偏瘫患者的疗效观察

Effect of Xingnao Kaiqiao acupuncture combined with electronic biofeedback in treating patients with hemiplegia after stroke

    目的 观察醒脑开窍针刺法联合电子生物反馈对脑卒中后偏瘫患者神经功能和运动功能的影响。
    方法 选取2024年7月—2025年7月在濮阳市中医医院康复科就诊的92例脑卒中后偏瘫患者为研究对象, 采用随机数字表法分为观察组与对照组,每组46例。对照组给予电子生物反馈治疗,观察组在此基础上加用醒脑开窍针刺治疗。比较2组临床疗效、中医证候积分、神经功能缺损量表(NDS)评分、腕关节和踝关节主动关节活动度(AROM)、Berg平衡量表(BBS)评分、Fugl-Meyer量表(FMA)评分、改良Barthel指数(MBI)以及肱二头肌、肱三头肌、胫前肌、腓肠肌表面肌电均方根值(RMS)。
    结果 观察组总有效率为95.65%, 高于对照组的80.43%, 差异有统计学意义(P < 0.05)。治疗后, 2组患者半身不遂、言语蹇涩、感觉减退、口舌歪斜、饮水发呛积分和总积分均较治疗前降低,且观察组降低幅度大于对照组,差异有统计学意义(P < 0.05)。治疗后,观察组和对照组NDS评分分别为(11.80±3.29)、(16.11±4.03)分,差异有统计学意义(P < 0.05)。治疗后, 2组患者腕关节和踝关节AROM均较治疗前增大,且观察组腕关节和踝关节AROM均大于对照组,差异有统计学意义(P < 0.05)。治疗后, 2组FMA、BBS、MBI评分均较治疗前提高,且观察组FMA、BBS、MBI评分均高于对照组,差异有统计学意义(P < 0.05)。治疗后,观察组和对照组肱二头肌、肱三头肌、胫前肌、腓肠肌的RMS均较治疗前升高,且观察组升高幅度大于对照组,差异有统计学意义(P < 0.05)。
    结论 醒脑开窍针刺联合电子生物反馈疗法,可有效改善脑卒中偏瘫患者的中医证候积分、神经功能缺损评分,提高运动功能,从而促进功能恢复,提高患者生活质量和治愈率。

     

    Abstract:
    Objective To observe the effects of Xingnao Kaiqiao acupuncture combined with electronic biofeedback on neurological and motor functions in patients with hemiplegia after stroke.
    Methods A total of 92 patients with hemiplegia after stroke in the Rehabilitation Department of Puyang Hospital of Traditional Chinese Medicine from July 2024 to July 2025 were selected as the study subjects, and they were randomly divided into an observation group and a control group, with 46 cases in each group. The control group received electronic biofeedback therapy, while the observation group received additional Xingnao Kaiqiao acupuncture therapy. Clinical efficacy, scores of traditional Chinese medicine syndromes, score of the Neurological Deficit Scale (NDS), active range of motion (AROM) of the wrist and ankle joints, score of Berg Balance Scale (BBS), score of Fugl-Meyer Assessment (FMA), score of modified Barthel index (MBI) as well as the root mean square (RMS) values of surface electromyography of the biceps brachii, triceps brachii, tibialis anterior, and gastrocnemius muscles were compared between the two groups.
    Results The total effective rate was 95.65% in the observation group, which was significantly higher than 80.43% in the control group (P < 0.05). After treatment, the scores for hemiplegia, slurred speech, hypoesthesia, facial deviation choking on drinking water, and the total scores in both groups decreased significantly compared to those before treatment, and the decrease was more pronounced in the observation group than in the control group (P < 0.05). After treatment, the NDS scores in the observation group and the control group were (11.80±3.29) and (16.11±4.03) points respectively, with a significant between-group difference (P < 0.05). After treatment, AROM of the wrist and ankle joints in both groups increased significantly compared to those before treatment, and the AROM of the wrist and ankle joints in the observation group were significantly higher than those in the control group (P < 0.05). After treatment, the FMA, BBS, and MBI scores in both groups increased significantly compared to those before treatment, and the FMA, BBS, and MBI scores in the observation group were significantly higher than those in the control group (P < 0.05). After treatment, the RMS values of the biceps brachii, triceps brachii, tibialis anterior, and gastrocnemius muscles in both groups increased significantly compared to those before treatment, and the increase was more pronounced in the observation group than in the control group (P < 0.05).
    Conclusion The combination of Xingnao Kaiqiao acupuncture and electronic biofeedback therapy can effectively improve scores of traditional Chinese medicine syndromes and neurological deficit scores, enhance motor function, thereby promoting functional recovery, improving the quality of life and cure rate in patients with hemiplegia after stroke.
