甘油三酯葡萄糖乘积指数联合甘油三酯/高密度脂蛋白胆固醇对缺血性脑卒中的预测价值

Predictive value of triglyceride-glucose index combined with triglyceride/high-density lipoprotein cholesterol for ischemic stroke

    目的 探讨甘油三酯葡萄糖乘积指数(TyG)与甘油三酯/高密度脂蛋白胆固醇(TG/HDL-C)对缺血性脑卒中发生的预测价值。
    方法 该研究为回顾性研究, 选择2023年12月—2025年1月在南京大学医学院附属鼓楼医院就诊的患者280例为研究对象。根据患者头颅磁共振成像结果,将患者分为脑卒中组(缺血性脑卒中, n=102)和对照组(无缺血性脑卒中, n=178)。比较2组基本资料、实验室检查结果、TyG、TG/HDL-C; 采用限制性立方样条(RCS)分析TyG和TG/HDL-C与脑卒中的相关性; 采用Logistic回归分析筛选脑卒中的影响因素; 采用受试者工作特征(ROC)曲线分析TyG、TG/HDL-C和二者联合对脑卒中的预测效能。
    结果 脑卒中组男性、吸烟、高血压、既往脑梗死史患者占比均高于对照组,总胆红素(TB)、直接胆红素(DB)、空腹血糖(FPG)、血肌酐(Cr)、甘油三酯(TG)、载脂蛋白A1、载脂蛋白B、TyG及TG/HDL-C均高于对照组(P < 0.05); RCS分析结果显示, TyG和TG/HDL-C均与脑卒中呈正相关。多因素Logistic回归分析显示, TyG和TG/HDL-C较高是脑卒中的独立危险因素。ROC曲线分析显示, TyG与TG/HDL-C联合检测对脑卒中的发病具有一定预测价值。
    结论 TyG、TG/HDL-C为缺血性脑卒中的独立危险因素,具有一定预测缺血性脑卒中的价值。

     

    Abstract:
    Objective To explore the predictive value of the triglyceride-glucose index (TyG) and triglyceride/high-density lipoprotein cholesterol (TG/HDL-C) for the occurrence of ischemic stroke.
    Methods This was a retrospective study. A total of 280 patients who sought medical treatment at Drum Tower Hospital Affiliated to Medical School of Nanjing University from December 2023 to January 2025 were selected as research subjects. According to the results of head magnetic resonance imaging, the patients were divided into stroke group (ischemic stroke, n=102) and control group (no ischemic stroke, n=178). The basic data, laboratory test results, TyG, and TG/HDL-C of the two groups were compared. Restricted cubic spline (RCS) analysis was used to analyze the correlations of TyG, and TG/HDL-C with stroke. Logistic regression analysis was employed to screen the influencing factors of stroke. Receiver operating characteristic (ROC) curve analysis was conducted to evaluate the predictive efficacy of TyG, TG/HDL-C, and their combination for stroke.
    Results The proportions of male patients, smokers, patients with hypertension, and patients with a previous history of cerebral infarction in the stroke group were all higher than those in the control group. The levels of total bilirubin (TB), direct bilirubin (DB), fasting plasma glucose (FPG), serum creatinine (Cr), triglycerides (TG), apolipoprotein B, TyG, and TG/HDL-C in the stroke group were all higher than those in the control group (P < 0.05). RCS analysis results showed that both TyG and TG/HDL-C were positively correlated with stroke. Multivariate Logistic regression analysis revealed that higher levels of TyG and TG/HDL-C were independent risk factors for stroke. ROC curve analysis indicated that the combined detection of TyG and TG/HDL-C had a certain predictive value for the onset of stroke.
    Conclusion TyG and TG/HDL-C are independent risk factors for ischemic stroke and have a certain predictive value for ischemic stroke.
