表皮生长因子受体和磷酸酶及张力蛋白同源物蛋白在慢性萎缩性胃炎中的表达及临床意义

Expression and clinical significance of epidermal growth factor receptor and phosphatase and tensin homolog protein in chronic atrophic gastritis

    目的 分析慢性萎缩性胃炎(CAG)患者胃黏膜组织中表皮生长因子受体(EGFR)、磷酸酶及张力蛋白同源物(PTEN)蛋白表达及其临床意义。
    方法 选择2022年3月—2023年12月在邯郸市中医院就诊的慢性胃炎患者183例为研究对象,其中慢性非萎缩性胃炎(CNAG)患者97例(CNAG组),CAG患者86例(CAG组)。收集研究对象临床资料。采用免疫组化法检测2组胃黏膜组织中EGFR、PTEN蛋白的表达情况; 分析CAG患者胃黏膜组织中EGFR和PTEN蛋白表达与临床病理特征的关系; 采用Logistic回归分析筛选CAG患者预后不良的影响因素。
    结果 CAG组幽门螺杆菌(Hp)感染患者占比、EGFR阳性表达率高于CNAG组,PTEN阳性表达率低于CNAG组,差异有统计学意义(P < 0.05)。EGFR、PTEN蛋白表达与CAG患者异型增生、Hp感染及预后良好密切相关(P < 0.05)。Logistic回归分析结果显示, EGFR阳性表达是影响CAG患者预后的危险因素, PTEN阳性表达是影响CAG患者预后的保护因素(P < 0.05)。
    结论 CAG患者胃黏膜组织中EGFR蛋白表达大多呈阳性, PTEN蛋白表达大多呈阴性,二者与胃黏膜肠上皮化生、异型增生、Hp感染及预后有关。

     

    Abstract:
    Objective To analyze the expression of epidermal growth factor receptor (EGFR) and phosphatase and tensin homolog (PTEN) proteins in gastric mucosa tissues of patients with chronic atrophic gastritis (CAG) and their clinical significance.
    Methods A total of 183 patients with chronic gastritis who visited Handan Hospital of Traditional Chinese Medicine from March 2022 to December 2023 were selected as the study subjects, including 97 patients with chronic non-atrophic gastritis (CNAG) (CNAG group) and 86 patients with CAG (CAG group). Clinical data of the study subjects were collected. Immunohistochemistry was used to detect the expression of EGFR and PTEN proteins in gastric mucosa tissues of the two groups. The relationship between the expression of EGFR and PTEN proteins in gastric mucosa tissues of CAG patients and their clinicopathological characteristics was analyzed. Logistic regression analysis was employed to screen for factors influencing poor prognosis in CAG patients.
    Results The proportion of Helicobacter pylori (Hp)-infected patients and the positive expression rate of EGFR in the CAG group were higher than those in the CNAG group, while the positive expression rate of PTEN was lower than that in the CNAG group, with statistical significance (P < 0.05). The expression of EGFR and PTEN proteins was closely related to dysplasia, Hp infection, and good prognosis in CAG patients (P < 0.05). Logistic regression analysis showed that positive EGFR expression was a risk factor affecting the prognosis of CAG patients, while positive PTEN expression was a protective factor affecting the prognosis of CAG patients (P < 0.05).
    Conclusion In the gastric mucosa tissues of CAG patients, EGFR protein expression is positive, and PTEN protein expression is negative, which is related to intestinal metaplasia, dysplasia, Hp infection, and prognosis of the gastric mucosa.
