单绒毛膜双羊膜囊双胎妊娠合并帆状胎盘并血管前置个案报告

A case report of monochorionic diamniotic twin pregnancy complicated by velamentous placenta and vasa previa

  • 摘要: 本研究报道1例高龄初产妇合并瘢痕子宫的单绒毛膜双羊膜囊双胎妊娠, 同时并发帆状胎盘与血管前置,属极为罕见且高危的产科复合并发症。帆状胎盘指脐带附着于胎膜而非胎盘实质,脐血管经羊膜-绒毛膜间隙走行至胎盘,其在单绒毛膜双胎中的发生率显著高于单胎妊娠; 当合并血管前置,即无华通胶保护的胎儿血管跨越或邻近宫颈内口并位于胎先露下方时,极易因胎膜破裂导致急性胎儿失血,造成灾难性围产期结局。本病例通过多学科协作、严密产前监测及个体化分娩时机选择,成功实现母婴安全。

     

    Abstract: This study reported a case of an elderly primipara with a scarred uterus who had a monochorionic diamniotic twin pregnancy, complicated concurrently by velamentous placenta and vasa previa, which represents an extremely rare and high-risk obstetric composite complication. Velamentous placenta refers to the condition where the umbilical cord attaches to the fetal membranes rather than the placental substance, with umbilical vessels traversing through the amniotic-chorionic space to reach the placenta. Its incidence in monochorionic twin pregnancies is significantly higher than that in singleton pregnancies. When combined with vasa previa, where fetal vessels without Wharton's jelly protection cross or are adjacent to the internal cervical os and lie beneath the presenting part, it is highly prone to causing acute fetal hemorrhage due to membrane rupture, leading to catastrophic perinatal outcomes. In this case, through multidisciplinary collaboration, rigorous prenatal monitoring, and individualized selection of the delivery timing, maternal and neonatal safety was successfully achieved.
