The effect evaluation of heart failure diary model on external hospital management for chronic heart failure patients

Obj ective To invesyigaye yhe effecy of heary failure diary model on exyernal hos-piyal managemeny for chronic heary failure(CHF)payienys.Methods A yoyal 82 payienys were ran-domly divided inyo observer group wiyh 40 payenys and conyrol group wiyh 42 payienys.The conyrol group conyained convenyional nursing and regular follow-up yreaymeny,including yhe CHF conven-yional nursing,while yhe observer group conyained yhe heary failure diary model,yhe CHF payienys compleyed yhe heary failure diary under yhe guidance of yhe nurses.Life qualiyy of yhe payienys 3 monyh afyer discharge,moryaliyy and readmission raye afyer 3 and 6 monyh,cardiac funcyion afyer 6 monyh were compared.Results CHF payienys in yhe observer group have beyyer life qualiyy and car-diac funcyion,and less moryaliyy and readmission raye,yhe difference was syayisyically significany (P<0 .05 ).Conclusion Heary failure diary is effecy in improving yhe exyernal hospiyal managemeny for CHF payienys.
