股深动脉重建治疗下肢动脉硬化闭塞症患者下肢缺血的疗效

Effect of deep femoral artery reconstruction on treatment of lower limb ischemia of patients with lower extremity arterial occlusive disease

  • 摘要: 目的:探讨股深动脉重建治疗下肢动脉硬化闭塞症(LEAOD)患者下肢缺血的临床疗效。方法选取 FontaineⅢ、Ⅳ期 LEAOD 患者45例,对患者应用股深动脉重建手术进行治疗,对治疗效果进行观察。结果临床有效率为95.6%,合并有溃疡和坏疽的患者中有超过70%的病例创面在5周内愈合,接受治疗的患者在随访期的血管通畅率均超过90%,本组患者治疗半年后的踝/肱指数(ABI)和间歇性跛行距离均较治疗前显著提高(P <0.05)。结论股深动脉重建手术治疗 FontaineⅢ、Ⅳ期 LEAOD 患者下股缺血疗效显著,可明显改善患者的临床症状,降低并发症的发生率,并可获得较高的远期血管通畅率。

     

    Abstract: Objective To explore the clinical effect of the deep femoral artery reconstruction on the treatment of the lower limb ischemia of the patients with lower extremity arterial occlusive disease (LEAOD).Methods 45 LEAOD patients with FontaineⅢ or Ⅳ phase were treated with the deep femoral artery reconstruction operation,and the therapeutic effect was observed.Results Clinical effective rate was 95.6%.More than 70% of the patients with ulcer and gangrene had wound healing in 5 weeks and the patency rate of the patients in the follow-up days exceeded 90%.The ankle brachial index and intermittent claudication distance of the patients at half a year after the treatment were significantly higher than those before the treatment (P <0.05).Conclu-sion The curative efficacy of the deep femoral artery reconstruction operation in the treatment of lower limb ischemia of the LEAOD patients in Fontaine Ⅲ or Ⅳ phase is significant.The treatment can improve the typical clinical symptoms significantly,reduce the risk of complications,and obtain a higher long-term patency rate.
