雷公藤多苷对老年原发性肾病综合征患者血清指标的影响

Effect of glycosides on serum indexes in elderly patients with primary nephrotic syndrome

  • 摘要: 目的 观察雷公藤多苷治疗老年原发性肾病综合征的疗效及其对患者血清疾病相关指标的影响.方法 将280例老年原发性肾病综合征患者随机分为对照组和观察组各140例,对照组采用西药常规治疗,观察组则加用雷公藤多苷治疗.比较2组疗效及治疗前后24 h尿蛋白定量、血清脂联素、白蛋白、血肌酐以及免疫学指标、糖类抗原水平.结果 观察组临床控制率和总有效率显著高于对照组(P<0.05).治疗后第4、8周,观察组24 h尿蛋白定量、血清脂联素、血肌酐显著低于对照组,而血浆白蛋白水平则显著高于对照组(P<0.05);观察组患者血清IgA 、IgG水平及IgG/IgM比值显著高于对照组,而IgM水平则显著低于对照组(P<0.05);观察组患者血清CA125、CA153、CA199水平显著低于对照组(P<0.05).结论 雷公藤多苷能明显提高老年原发性肾病综合征的临床缓解率,减少白蛋白的丢失,保护肾功能,延缓NS病情进展.

     

    Abstract: Objective To observe the effect of glycosides on serum indexes in elderly patients with primary nephrotic syndrome.Methods A total of 280 elderly patients with primary nephrotic syndrome were randomly divided into control group and observation group,140 cases in each group.Patients in the control group were treated with conventional western medicine,and those in the observation group were treated with additional glycosides.The efficacy and 24 h urinary protein,serum adiponectin,serum albumin,serum creatinine and immunological parameters,carbohydrate antigen levels before and after treatment were compared between two groups.Results The clinical control rate and total effective rate in the observation group were significantly higher than those in the control group (P < 0.05).4,8 weeks after treatment,24 h urinary protein,serum adiponectin,serum creatinine of observation group were significantly lower than those of the control group,while the serum albumin was significantly higher than the control group (P < 0.05).The serum levels of IgA,IgG and IgG/IgM in the observation group were significantly higher than those in the control group,while the IgM level was significantly lower than that in the control group (P < 0.05).The serum levels of CA125,CA153 and CA199 in the observation group were significantly lower than those in the control group (P < 0.05).Conclusion Glycosides can effectively increase the clinical remission rate of primary nephrotic syndrome in elderly patients,reduce the loss of albumin and protect renal function,and delay the progression of NS.
