超声引导下微创旋切术治疗乳腺良性肿瘤的手术效果和美观效果评价

Evaluation on therapeutic efficacy and aesthetic effect of ultrasound-guided minimally invasive surgery for benign breast tumors

      目的  探讨乳腺良性肿瘤行超声引导下微创旋切术治疗的手术效果和美观效果。
      方法  选取本院乳腺外科收治的80例乳腺良性肿瘤患者,均在本院接受外科切除手术。根据随机分配原则和患者意愿分成2组,其中38例接受传统切除手术的为传统组, 42例接受超声引导下微创旋切术治疗的为微创组。比较2组手术临床指标、视觉模拟疼痛(VAS)评分、并发症发生率和手术美观效果。术后1年随访比较2组复发情况。
      结果  与传统组比较,微创组手术时间、切口愈合时间较短,出血量较少,术后1 d VAS评分显著较低,瘢痕长度显著较短(P < 0.01); 微创组手术并发症率为9.52%, 低于传统组的28.95%, 美观效果优良率为95.24%, 显著高于传统组的76.32%, 差异有统计学意义(P < 0.05); 传统组随访1年复发1例,微创组无复发患者, 2组1年复发率无显著差异(P>0.05)。
      结论  超声引导下微创旋切术治疗乳腺良性肿瘤不仅创伤小,并发症少,而且术后乳房美观效果更佳。

     

    Abstract:
      Objective  To investigate the surgical and aesthetic effect of ultrasound-guided minimally invasive surgery for benign breast tumors.
      Methods  A total of 80 patients with benign breast tumors admitted to our hospital underwent excision surgeries. According to the principle of random allocation and patients' willingness, they were divided into two groups. The traditional group(n=38) was given traditional resection surgery, and minimally invasive surgery group(n=42) were treated by ultrasound-guided minimally invasive rotary excision. The clinical indicators, VAS scores, incidence of complications and aesthetic effect of the two groups were compared. The recurrence condition of the two groups was compared after 1 year follow-up.
      Results  The operation time, incision healing time was earlier, bleeding volume, visual analogue pain (VAS) score was less and scar length was shorter in minimally invasive surgery group compared to traditional group (P < 0.01); the complication rate of minimally invasive surgery group was 9.52%, which was lower than 28.95% of the traditional group, and the aesthetic effect rate was 95.24%, which was higher than 76.32% of traditional group(P < 0.05). There was one recurrence case in the traditional group and no recurrence case in the minimally invasive group. No significant difference in one-year recurrence rate between the two groups was seen(P>0.05).
      Conclusion  Ultrasound-guided minimally invasive surgery in the treatment of benign breast tumors not only has the advantages of less complications and trauma, but also has better aesthetic effect after operation.
