尺桡骨骨折患者维生素D、骨源性碱性磷酸酶含量与骨折愈合速度的相关性研究

Correlation between vitamin D, bone alkaline phosphatase contents and fracture healing speed in patients with radial and ulnar fractures

  • 摘要: 目的 探讨尺桡骨骨折患者维生素D、骨源性碱性磷酸酶(BALP)含量与骨折愈合速度的相关性。 方法 回顾性分析90例尺桡骨骨折患者临床资料,术前及术后5、14 d检测25-羟基维生素D3[25(OH)D3]及BALP含量。根据术前检测结果,从中抽取26例低于平均值20%的患者作为对照组,抽取26例高于平均值20%的患者作为观察组。术后1、3个月时拍摄X线片,观察患者骨折愈合情况,分析尺桡骨骨折患者维生素D、BALP含量与骨折愈合速度的相关性。 结果 患者术前及术后5、14 d时的25(OH)D3及BALP水平呈显著下降趋势(P<0.05)。观察组术前25(OH)D3及BALP水平均显著高于对照组(P<0.01)。观察组骨痂形成时间及骨折线消失时间均显著短于对照组(P<0.05)。Pearson相关性分析显示,患者维生素D、BALP含量与骨痂形成时间及骨折线消失时间呈显著负相关(r=-0.687、-0.390、-0.806、-0.548, P=0.018、0.002、0.003、0.037)。 结论 维生素D、BALP含量与骨折愈合速度有关,可通过补充外源性维生素D等方案来缩短患者骨折愈合时间。

     

    Abstract: Objective To investigate the correlation between vitamin D, bone alkaline phosphatase(BALP)contents and fracture healing speed in patients with radial and ulnar fractures. Methods The clinical materials of 90 patients with radial and ulnar fracture were retrospectively analyzed, and the contents of 25-dihydroxyvitamin D3 [25(OH)D3] and BALP were detected before surgery, 5 and 14 days after surgery. Based on the preoperative detection results, 26 patients with results less than 20% of the average value were taken as control group, and the patients with results over than 20% of the average value were taken as observation group. X-ray films were taken at 1 month and 3 months after surgery, and the fracture healing conditions of the patients were observed. The correlations between vitamin D, BALP contents and fracture healing speed in patients with ulnar and radial fracture were analyzed. Results The contents of 25(OH)D3 and BALP decreased gradually before surgery and 5, 14 days after surgery in all the patients, and there were significant differences(P<0.05). The contents of 25(OH)D3 and BALP in the observation group were significantly higher than those in the control group(P<0.01). The time of callus formation and fracture line disappearance in the observation group was significantly shorter than that in the control group(P<0.05). - Pearson correlation analysis showed that there were significant negative correlations between vitamin D, BALP contents and callus formation time, fracture line disappearance time(r=-0.687, -0.390, -0.806, -0.548, P=0.018, 0.002, 0.003, 0.037). Conclusion Vitamin D and BALP contents are related to fracture healing speed. The supplementation of exogenous vitamin D can shorten the healing time of fractures.
