地屈孕酮联合人绒毛膜促性腺激素对先兆流产的疗效观察

Effect of dydrogesterone and human chorionic gonadotropin in the treatment of threatened abortion

  • 摘要: 目的 观察地屈孕酮联合人绒毛膜促性腺激素(HCG)对先兆流产的治疗效果及对妊娠相关蛋白A(PAPP-A)、胎儿纤维连接蛋白(fFN)、抑制素A的影响。 方法 选取先兆流产孕妇180例为研究对象,按随机投掷法分为对照组和研究组。对照组采用黄体酮联合HCG治疗,研究组采用地屈孕酮联合HCG治疗,比较2组的治疗效果、不良反应情况和治疗前后PAPP-A、fFN、抑制素A、孕酮、雌二醇水平。 结果 研究组的治疗有效率为90.00%, 高于对照组的76.67%, 差异有统计学意义(P<0.05); 2组不良反应发生率比较差异无统计学意义(P>0.05); 治疗后,研究组PAPP-A、抑制素A水平高于对照组, fFN水平低于对照组,差异有统计学意义(P<0.05); 治疗前, 2组孕酮、雌二醇水平比较,差异无统计学意义(P>0.05); 治疗后,研究组孕酮、雌二醇水平高于对照组,差异有统计学意义(P<0.05)。 结论 地屈孕酮联合HCG治疗先兆流产的疗效显著,可升高孕妇PAPP-A、抑制素A、孕酮、雌二醇水平,降低fFN水平。

     

    Abstract: Objective To observe the therapeutic effect of dydrogesterone and human chorionic gonadotropin(HCG)in the treatment of threatened abortion and its effect on pregnancy associated protein A(APAPP-A), fetal fibronectin(fFN)and inhibin A. Methods A total of 180 threatened abortion women were selected and divided into control group and study group according to the random throwing method. The control group was treated with progesterone combined with HCG, and the study group was treated with dydrogesterone combined with HCG. The treatment effects, adverse reactions, PAPP-A, fFN, inhibin A levels, progesterone, and estradiol levels before and after treatment were compared between the two groups. Results The effective rate of treatment in the study group was 90.00%, which was higher than 76.67% in the control group, the difference was significant(P<0.05); the incidence of adverse reactions showed no significant differences between the two groups(P>0.05); after treatment, the levels of PAPP-A and inhibin A in the study group were higher, and the fFN level was significantly lower than that in the control group(P<0.05); the levels of progesterone and estradiol before treatment in the two groups showed no significant differences(P>0.05), but were significantly higher in the study group after treatment than those of the control group(P<0.05). Conclusion Dydrogesterone and HCG in combination have a significant therapeutic effect on threatened abortion, and can increase the levels of PAPP-A, inhibin A, progesterone and estradiol, and decrease the level of fFN.
