非酒精性脂肪性肝病住院患者心理健康状况及其影响因素

Mental health status of inpatients with nonalcoholic fatty liver disease and its influencing factors

    目的 了解非酒精性脂肪性肝病(NAFLD)住院患者的心理健康状况,并分析其影响因素。
    方法 选取NAFLD住院患者及主要照顾者各145例,采用一般资料调查表、广泛性焦虑量表(GAD-7)、患者健康抑郁问卷量表(PHQ-9)、照顾者负担量表(ZBI)对患者及其主要照顾者进行调查。
    结果 NAFLD患者中,焦虑患者71例(48.97%),抑郁患者94例(64.83%)。单因素分析结果显示,有焦虑的患者文化程度、医疗费用支付方式及主要照顾者有无照顾负担与非焦虑患者比较,差异有统计学意义(P<0.05);有抑郁患者的文化程度、医疗费用支付方式、有无工作及主要照顾者的文化程度、是否有娱乐活动、照顾者有无负担与非抑郁患者比较,差异有统计学意义(P<0.05)。二分类Logistic回归分析结果显示,照顾者有负担是患者焦虑的危险因素(P<0.05);患者采用新型农村合作医疗支付医疗费用、无工作及主要照顾者文化程度低(小学及中学)、照顾者有负担是NAFLD住院患者抑郁的危险因素(P<0.05)。
    结论 NAFLD住院患者心理健康状况不佳,合并焦虑、抑郁的患者占比较高,除患者因素外,主要照顾者因素也是其主要影响因素。

     

    Abstract:
    Objective To understand the mental health status of hospitalized patients with nonalcoholic fatty liver disease (NAFLD) and analyze its influencing factors.
    Methods A total of 145 hospitalized patients with NAFLD and 145 their main caregivers were selected. General information questionnaire, 7-item Generalized Anxiety Disorder Questionnaire (GAD-7), Patient Health Questionnaire-9 (PHQ-9), Zarit Caregiver Burden Interview (ZBI) were used to investigate patients and their caregivers.
    Results Among NAFLD patients, 71 patients (48.97%) had anxiety and 94 patients (64.83%) had depression. The results of univariate analysis showed that the differences in patient's education, medical payment method and primary caregiver's caregiver burden were statistically significant in patients with and without anxiety (P<0.05); the differences in patient's education, medical payment method, availability of work and primary caregiver's education, recreational activities and caregiver burden were statistically significant in patients with and without depression (P<0.05). Binary Logistic regression analysis showed that burden of caregivers was a risk factor for patients' anxiety (P<0.05); the risk factors for depression in NAFLD inpatients were the payment of medical expenses by the new rural cooperative medical system, unemployment, the low education level of the primary caregivers (primary and secondary school), and the burden of caregivers (P<0.05).
    Conclusion NAFLD inpatients with poor mental health status and a high occurrence of anxiety and depression, and caregiver factors are also the main influencing factors.
