慢性心力衰竭患者静息心率与血清超敏C反应蛋白、脑钠肽及左室射血分数的相关性

Correlation between resting heart rate and serum high sensitive C reactive protein, brain natriuretic peptide and left ventricular ejection fraction in patients with chronic heart failure

      目的  探讨慢性心力衰竭(CHF)患者静息心率(RHR)与血清超敏C反应蛋白(hs-CRP)、脑钠肽(BNP)及左室射血分数(LVEF)的相关性。
      方法  选取120例慢性心力衰竭患者,根据RHR分为A组37例(RHR < 70次/min)、B组45例(RHR为70~90次/min)和C组38例(RHR>90次/min)。入院时检测并比较3组血清hs-CRP、BNP、LVEF水平。采用Pearson法分析RHR与血清hs-CRP、BNP、LVEF水平的相关性。
      结果  C组纽约心脏学会(NYHA)分级Ⅲ~Ⅳ级患者比例显著高于A组、B组(P < 0.05)。A组、B组、C组血清hs-CRP、BNP水平依次升高, LVEF依次下降,组间两两比较,差异均有统计学意义(P < 0.05)。Pearson法分析显示, CHF患者RHR与血清hs-CRP、BNP均呈显著正相关(r=0.493、0.572, P=0.034、0.027), 与LVEF呈显著负相关(r=-0.518, P=0.031)。
      结论  CHF患者RHR与hs-CRP、BNP水平呈显著正相关,与LVEF呈显著负相关。

     

    Abstract:
      Objective  To investigate the correlation between resting heart rate (RHR) and serum high-sensitivity C reactive protein (hs-CRP), brain natriuretic peptide (BNP) and left ventricular ejection fraction (LVEF) in patients with chronic heart failure (CHF).
      Methods  A total of 120 patients with CHF were divided into group A with RHR less than 70 times per minute (n=37), group B with RHR from 70 to 90 times per minute (n=45) and group C with RHR over 90 times per minute (n=38). The levels of hs-CRP, BNP and LVEF were measured and compared among three groups. Pearson method was used to analyze the correlation between RHR and hs-CRP, BNP and LVEF.
      Results  The proportion of patients with grade Ⅲ to Ⅳ of New York Heart Association (NYHA) in group C was significantly higher than that in group A and group B (P < 0.05). The levels of hs-CRP and BNP in group A, B and C increased in proper order, while LVEF decreased in proper order, and there were significant differences among three groups (P < 0.05). Pearson analysis showed that RHR was positively correlated with hs-CRP and BNP (r=0.493, 0.572, P=0.034, 0.027) and negatively correlated with LVEF (r=-0.518, P=0.031).
      Conclusion  RHR is positively correlated with hs-CRP, BNP and negatively correlated with LVEF in CHF patients.
