超声造影联合乳腺钼靶X线对乳腺导管内乳头状瘤的诊断价值

Diagnostic value of contrast-enhanced ultrasound combined with molybdenum target X-ray of breast for intraductal papilloma

      目的  探讨超声造影(CEUS)联合乳腺钼靶X线对乳腺导管内乳头状瘤(IDP)的诊断价值。
      方法  回顾性分析80例乳腺导管内病变患者临床资料,均予以CEUS、乳腺钼靶X线检查。评估2种方法单独应用和联合应用的检出率、诊断准确率,并分析其受试者工作特性(ROC)曲线。
      结果  CEUS诊断准确率为77.50%;乳腺钼靶X线检查诊断准确率为70.00%;CEUS联合乳腺钼靶X线诊断准确率为96.25%。ROC曲线分析显示,CEUS、乳腺钼靶X线、CEUS联合乳腺钼靶X线诊断IDP的曲线下面积(AUC)分别为0.601、0.607、0.663;CEUS联合乳腺钼靶X线诊断的敏感性、特异性均分别高于CEUS、乳腺钼靶X线,差异有统计学意义(P < 0.05)。
      结论  CEUS联合乳腺钼靶X线检查对IDP具有较高诊断价值。

     

    Abstract:
      Objective  To explore the diagnostic value of contrast-enhanced ultrasound (CEUS) combined with molybdenum target X-ray of breast for intraductal papilloma(IDP).
      Methods  The clinical data of 80 patients with breast intraductal lesions was retrospectively analyzed. All patients were examined by CEUS, and molybdenum target X-ray of breast. The detection rate and accuracy rate of CEUS, molybdenum target X-ray of breast and their combination were evaluated, and their receiver operating characteristic (ROC) curve was analyzed.
      Results  The accuracy rate of CEUS was 77.50%; the accuracy rate of molybdenum target X-ray of breast was 70.00%; the accuracy rate of CEUS combined with molybdenum target X-ray of breast was 96.25%. ROC curve analysis showed that area under the curve (AUC) of CEUS, molybdenum target X-ray of breast and CEUS combined with molybdenum target X-ray of breast for IDP diagnosis were 0.601, 0.607 and 0.663, respectively, and the sensitivity and specificity of CEUS combined with molybdenum target X-ray of breast were significantly higher than those of CEUS and molybdenum target X-ray of breast alone (P < 0.05).
      Conclusion  CEUS combined with molybdenum target X-ray of breast has high diagnostic value for IDP detection.
