老年骨质疏松性骨折患者围术期抑郁发生情况及影响因素

Occurrence of perioperative depression in elderly patients with osteoporotic fractures and its influencing factors

      目的  探讨老年骨质疏松性骨折患者围术期抑郁发生情况及影响因素。
      方法  采取便利抽样法选取老年骨质疏松性骨折手术患者200例为研究对象。调查患者抑郁发生情况和影响因素,分析各影响因素与抑郁发生的相关性。
      结果  术前发生抑郁的患者为64例,占比为32.00%(64/200);术后7 d发生抑郁的患者为56例,占比为28.00%(56/200);术后14 d发生抑郁的患者为37例,占比为18.50%(37/200)。性别、营养状况、衰弱、社会与家庭支持情况以及睡眠情况是老年骨质疏松性骨折患者抑郁的影响因素(P < 0.05)。性别、社会与家庭支持、睡眠障碍、存在营养风险以及衰弱均与骨质疏松性骨折患者抑郁呈正相关(P < 0.05)。
      结论  老年骨质疏松性骨折手术患者术前及术后7 d产生抑郁情绪的患者显著多于术后14 d。社会与家庭支持、营养风险、身体衰弱、性别以及睡眠障碍是影响患者抑郁的主要因素。

     

    Abstract:
      Objective  To investigate the occurrence of perioperative depression in senile osteoporotic fractures and its influencing factors.
      Methods  Two hundred elderly patients with osteoporotic fractures were selected as research objects by convenience sampling. Occurrence and influencing factors of depression in patients were investigated, the correlations between the factors and the occurrence of depression were analyzed.
      Results  Preoperative depression occurred in 64 patients, accounting for 32.00%(64/200); depression occurred in 56 patients 7 d after operation, accounting for 28.00%(56/200); depression occurred in 37 patients 14 d after operation, accounting for 18.50%(37/200). Gender, nutritional status, weakness, social and family support status and sleep status were the influencing factors of depression in elderly patients with osteoporotic fractures (P < 0.05). Gender, social and family support, sleep disturbance, nutritional risk and weakness were positively correlated with depression in patients with osteoporotic fractures (P < 0.05).
      Conclusion  The incidence of depression in aged patients with osteoporotic fractures is significantly higher before surgery and 7 d after surgery than that of 14 d after surgery.Social and family support, nutritional risk, weakness, gender, and sleep disorders are the main factors affecting depression.
