2型糖尿病患者性别差异与冠状动脉血流储备受损的相关性分析

Correlation between gender difference and impaired coronary flow reserve in patients with type 2 diabetes mellitus

    目的 探讨2型糖尿病患者冠状动脉血流储备(CFR)受损的性别差异。
    方法 选取2型糖尿病患者201例(排除阻塞性冠心病者)为研究对象, 分为男性组(n=97)和女性组(n=104)。分析2组CFR及临床指标的差异; 应用多元Logistic回归分析模型探讨2型糖尿病患者CFR的危险因素; 应用Pearson直线相关分析评估临床指标与CFR的相关性。
    结果 女性组CFR低于男性组, 血浆醛固酮水平、二尖瓣口舒张早期血流的峰值速度与二尖瓣环组织多普勒舒张早期的峰值速度比值(E/e')高于男性组, 差异有统计学意义(P < 0.05)。女性为糖尿病患者CFR相关的危险因素(OR=3.219, P=0.018), 而低醛固酮水平、低E/e'为糖尿病患者CFR相关的保护性因素(OR=0.568, P=0.032;OR=0.372, P=0.012)。糖尿病患者血浆醛固酮水平与CFR呈负相关(OR=-0.683, P=0.035), E/e'与CFR呈负相关(OR=-0.751, P=0.023)。
    结论 2型糖尿病患者中女性CFR受损较男性显著, 女性血浆醛固酮水平、E/e'升高可能是导致女性CFR受损的病因之一。

     

    Abstract:
    Objective To investigate the gender difference of impaired coronary flow reserve (CFR) in patients with type 2 diabetes mellitus.
    Methods A total of 201 patients with type 2 diabetes mellitus (excluding patients with obstructive coronary heart disease) were selected as research objects, and divided into male group (n=97) and female group (n=104).Differences in CFR and clinical indexs of the two groups were analyzed; the risk factors of CFR in type 2 diabetes patients were analyzed by multivariate Logistic analysis; the correlations between clinical indicators and CFR were evaluated by Pearson linear correlation analysis.
    Results CFR of the female group was significantly lower than that of the male group, and plasma aldosterone level as well as ratio of peak flow velocity in early diastolic mitral orifice to peak flow velocity in early diastolic mitral annulus doppler tissue (E/e') were significantly higher than those of the male group (P < 0.05).Female is risk factor associated with CFR in diabetes (OR=3.219, P=0.018), while low aldosterone level and low E/e'were protective factors associated with CFR in diabetes (OR=0.568, P=0.032;OR=0.372, P=0.012).Plasma aldosterone level was negatively correlated with CFR (OR=-0.683, P=0.035), E/e'was negatively correlated with CFR (OR=-0.751, P=0.023).
    Conclusion CFR is significantly impaired in female patients with type 2 diabetes than in males, and the increase of plasma aldosterone level and E/e'in females may be one of causes of CFR impairment in female.
